Greetings Parents and Students:

Welcome to Langston Hughes High School! We are very excited to open this new school with you. We are confident that LHHS will challenge you academically and prepare you for whatever your aspirations are after graduation.

Documents REQUIRED For Enrollment

From Previous School:

• Withdrawal Form from Previous School (if transferring during the school year)
• Complete Transcript
• Attendance Records
• Discipline Records
• Complete Mailing Address, Phone and Fax Numbers of Last School Attended
• Talented & Gifted Services/IEP/Ed. Psych Evaluation/Any special services or programs

From Home:  (Copies are acceptable)

• Certified Copy of Birth Certificate
• Student’s Social Security Card
• Immunization Records – Eye, Ear and Dental Certificate (GA Form 3300) AND Georgia Certificate of Immunization (GA Form 3231)
• Proof of Residence – Must present TWO (2) documents, no exceptions**
• Proof of Guardianship (If student does not live with both parents)
• Identification of Parent or Legal Guardian

**Acceptable Documentation
Copy of home mortgage payment book or Apartment Lease showing name of lessee, Homeowner’s/Renters insurance registration card, Current Utility Bill, Current bank statement, copy of home contract, receipt to have utilities connected, and current paycheck stub.
